Conflict of Interest Form

The Quad Cities Community Foundation staff and volunteers will, from time to time, face possible conflicts of interest or situations in which the appearance of conflict of interest could be detrimental to the Community Foundation and the communities it serves. The Community Foundation has adopted Conflict of Interest, Code of Ethics, and Confidentiality policies in recognition of its responsibility to maintain public trust and to meet requirements for filing its IRS Form 990 and National Standards Certification.

A conflict of interest is any situation where personal interests, including those of an immediate family member, friend or associate, could harm an individual's ability to make sound, objective decisions on the Quad Cities Community Foundation’s behalf. This includes situations in which it could reasonably appear personal interests have improper influence related to decisions and activities to which their QCCF service would apply.

The Community Foundation collects conflict of interest forms from all staff and volunteers annually.
Failure to complete and sign this form within 30 days of receiving notice to complete may result in being disqualified from participation.

However, this information is helpful for the Community Foundation in achieving our strategic goals and commitment to Diversity, Equity and Inclusion.
